  • Purpose As information on patients' preferences regarding quality-of-life aspects in sarcopenia is lacking, this study aims to assess the relative importance of the 14 items of a QoL questionnaire designed for sarcopenia (the SF-SarQoL) using a best-worst scaling (BWS) survey. Methods Participants, aged 65 years or older and community dwelling, who previously participated in the SarcoPhAge study, received a BWS survey via the mail. An object case BWS was selected in which participants completed 12 choice tasks, picking the most and least important aspect from 4 out of 14 SF-SarQoL items for each task. Relative importance scores (RIS) were estimated using Hierarchical Bayes modelling. A cluster analysis was also conducted to investigate whether several profiles with regards to QoL preferences were present. Results A total of 163 participants were included, aged 75 (IQR: 73-81) years old, and mostly women (n = 107; 65.6%). Two items were found to be significantly more important than others: "feeling a reduction of physical capacity" (RIS = 11.26), and "having balance problems" (RIS = 11.09). The least important items were "experiencing difficulty carrying heavy objects" (RIS = 2.89), and "feeling a reduction in muscle mass" (RIS = 3.82). We found relatively weak evidence for the presence of two clusters. One cluster prioritized items related to falls where the second prioritized items related to feeling physically capable.   • Background The global prevalence of diseases transmitted by Aedes aegypti mosquitoes, such as dengue, Zika and Yellow Fever, is increasing, but development of promising new mosquito control technologies could reverse this trend. Target Product Profiles (TPPs) and Preferred Product Characteristics (PPCs) documents issued by the World Health Organization can guide the research and development pathways of new products and product combinations transitioning from proof of concept to operational use. Methods We used high resolution global maps of the case and economic burden of dengue to derive programmatic cost targets to support a TPP for Wolbachia replacement. A compartmental entomological model was used to explore how release size, spacing and timing affect replacement speed and acceptability. To support a PPC for a hybrid suppress-then-replace approach we tested whether Wolbachia replacement could be achieved faster, more acceptably or at a lower cost if preceded by a mosquito suppression programme. Results We show how models can reveal trade-offs, identify quantitative thresholds and prioritise areas and intervention strategies for further development. We estimate that for Wolbachia replacement to be deployable in enough areas to make major contributions to reducing global dengue burden by 25% (in line with 2030 WHO targets), cost must ultimately be reduced to between $7.63 and $0.24 (USD) per person protected or less.   • Short-term mammalian toxicity studies combined with transcriptomics (TGx) can inform target organ effects and may further reduce subsequent testing and animal use. The goal of the present study was to assess added value of including TGx in a short-term (14 d), repeat-dose dietary study of two chemically-similar active substances in research and development, herein referred to herbicides C1 and C2. Male and female rats were exposed to 0, 600, 2000, 6000, 12000 ppm C1 (n=4/sex/grp; 0-548 mg/kg bw/day) and 0, 200, 600, 2000, 6000 ppm C2 (n=4/sex/grp; 0-451mg/kg bw/day). Daily endpoints included body weight, food consumption, clinical signs; terminal analyses included blood clinical chemistry, gross necropsy and weights/microscopic analyses of select organs. Animals exposed to C1 (all doses) had decreased body weight gain and those in the 6000 and 12000 ppm groups were terminated at 4d due to observed toxicity and excluded from TGx analysis. The adrenals and kidneys from animals exposed to C1 and C2 were analyzed with the TempO-Seq Whole Transcriptome Assay (BioClavis). Differentially expressed genes (DEGs: p=1.8 fold more DEGs than females. C2 DEGs also mapped to mitochondrial signaling pathways for both adrenals and kidneys. C1 had pathway effects in aldosterone signaling in the male adrenal only.   • The stability and resilience of the Earth system and human well-being are inseparably linked1–3, yet their interdependencies are generally under-recognized; consequently, they are often treated independently4,5. Here, we use modelling and literature assessment to quantify safe and just Earth system boundaries (ESBs) for climate, the biosphere, water and nutrient cycles, and aerosols at global and subglobal scales. We propose ESBs for maintaining the resilience and stability of the Earth system (safe ESBs) and minimizing exposure to significant harm to humans from Earth system change (a necessary but not sufficient condition for justice)4. The stricter of the safe or just boundaries sets the integrated safe and just ESB. Our findings show that justice considerations constrain the integrated ESBs more than safety considerations for climate and atmospheric aerosol loading. Seven of eight globally quantified safe and just ESBs and at least two regional safe and just ESBs in over half of global land area are already exceeded. We propose that our assessment provides a quantitative foundation for safeguarding the global commons for all people now and into the future. We find that justice considerations constrain the integrated Earth system boundaries more than safety considerations for climate and atmospheric aerosol loading, and our assessment provides a foundation for safeguarding the global commons for all people.
